I) Plaintiff, Jeff Davis, is a resident of Tulsa County, Oklahoma, at all times material to the events described herein.
2) Defendant, Preston Treadwell is a resident of Tulsa County, Oklahoma, and was at all times material to the events described herein.
3) That the events described herein occurred in and around 129°’ Street between 41SE and 51st Street in Tulsa, Oklahoma, located in Tulsa County, Oklahoma, and for this reason and the foregoing paragraphs, this Honorable Court has jurisdiction.
4) On or about April 21, 2012, the vehicle being operated by Defendant, Preston Treadwell, was driving northbound on 1291h Street when he failed to slow for traffic and collided with Plaintiff, Jeff Davis’s vehicle in the rear.
5) Defendant, Preston Treadwell, was operating his motor vehicle in an unsafe and
unreasonable manner by failing to slow for traffic in front of him causing his vehicle •o collide with the rear of Plaintiff’s vehicle.
6) Defendant, Preston Treadwell, failed to operate his vehicle as a reasonable prudent person would have which caused his vehicle to strike Plaintiff’s vehicle in the rear.
7) As a result of the events on or about April 21, 2012, Plaintiff, Jeff Davis has suffered injuries that have prevented him from transacting and conducting his business, and has suffered great pain of body and mind and incurred and continues to incur expenses for medical attention.

Outcome: Settled and dismissed with prejudice.
