Oklahoma Orthopedic Institute - $2,225.56

OKHCA Soonercare - $161.67

Total: $12,023.63

5. Plaintiffs attorney has a 50% contingency attorney fee contract, or $7,775.00 and costs of $1,735.97 which is superior to any claims of the medical lien holders;

WHEREFORE, premises considered, Plaintiff, Michael Robinson requests this Court enter an Order adjudicating all medical liens and other claims and apportion the settlement funds as requested by Plaintiff herein and for such other and further relief as this Honorable Court deems just and proper.

Outcome: The Court being well advised in the facts and circumstances of the case, and having reviewed the pleadings on, and after hearing argument of Counsel determines, finds and orders as follows:

1. The Court finds that notice of this hearing was served on all medical providers and/or lien holders by certified mail, and approved such service.

2. Plaintiff Michael Robinson recovered $15,500.00 from Farmers Insurance
Group.


The Court finds the settlement funds for Plaintiff Michael Robinson should be distributed as follows:

a. Attorney fees under a 50% contingent lien contract $ 6,88.02 and costs of $1,735.97.

b. OHCA $161.67

c. Associates Orthopedics - $2,289

d. Advanced Chiropractic - $3,302.00

e. Oklahoma Orthopedic Institute - $0

f. Stillwater Medical Center - $0

g. CBO Physician Clinic -$0

h. Stillwater Radiology - $0

i. City of Stillwater Amublance - $0

j. Blackwell Family Chiropractic - $125.00

K. Plaintiff $1,004.35

4. It is further ordered by the Court the signature of the non appearing medical providers on the settlement check may be endorsed by Plaintiffs attorney office.
