3. On September 15, 2009, and thereafter, the Plaintiff was a patient of thu Defendant. At all times mentioned herein, the Defendant Lars 0. Bouma DDS, MS, was acting individually and in his sole capacity as agent, servant or employee within the scope and course of his employment for Lars 0. Bouma DDS, PC.

4. On September 15, 2009 the Defendant performed orai surgery on Plainliff. As a result of the Defendant’s act the procedure made her jaw “fall in”, leadiDg to other severe complications including infection and excruciating pain.

5. Plaintiff states that her injuries resulted from the Defendant’s acts as follows:

a. The diagnosis, care and treatment rendered to the Plaintiff by the Defendant fell below acceptable professional dental standards; all as set out in the attached Exhibit A incorporated herein by reference.

b. That the Defendant failed to inform the Plaintiff of the condition of her mouth prior to the surgery and in all regards failed to inform her of the risk and consequences of the surgery as regarding her particular circumstance. Consequently, there was no informed consent given by the Plait itiff to the Defendant.

c. As a result of the uninformed consent to perform the surgery, the Defendant committed a battery upon the person of the Plaintiff.

d. As a result of the Defendant’s acts, Plaintiff requires ongoing dental treatment, has suffered excruciating pain and suffering and contnues to do so. She has suffered a disability of her person effecting her quality of life and well being and continues to do so.
